Preoperative Malnutrition Negatively Correlates With Postoperative Wound Complications and Infection After Total Joint Arthroplasty: A Systematic Review and Meta-Analysis

Gu A et al. • The Journal of arthroplasty (2019)

There is strong evidence that serologic malnutrition was associated with increased risk of poor postoperative outcome across all total joint replacement interventions.

  • Eleven (55%) studies used the total lymphocyte count, and 6 (30%) studies reported transferrin as a marker for malnutrition.
  • Among 20 studies, 18 (90%) studies reported a correlation with at least one serological marker and poor postoperative outcomes.
  • Finally, patients with an albumin level <3.5 dg/L were more likely to develop a postoperative wound complication (odds ratio: 2.176; 95% confidence interval: 1.916-2.471).

[Development of a Multi-functional Nano-device for Safe and Effective Gene Delivery to Target Organs]

Kodama Y • Yakugaku zasshi : Journal of the Pharmaceutical Society of Japan (2016)

The PyTAM-DGL-γ-PGA complexes markedly improved survival time of model mice infected with malaria.

  • The pDNA-DGL-γ-PGA complexes also had high gene expression in the marginal zone (rich dendritic cells) of the spleen after intravenous injection into mice.
  • These results indicate that pDNA-DGL-γ-PGA complexes may be useful as vaccine vectors.
  • Therefore we prepared a novel malaria DNA vaccine using Plasmodium yoelii GPI8p-transamidase-related protein pDNA (PyTAM).

Validation of Antibody-Based Strategies for Diagnosis of Pediatric Celiac Disease Without Biopsy

Wolf J et al. • Gastroenterology (2017)

In a prospective study, we validated the TTG-IgA procedure and the TTG-DGL procedure in identification of pediatric patients with or without celiac disease, without biopsy.

  • Based on our extrapolation model, we estimated that the PPV and NPV would remain >0.95 even at a disease prevalence as low as 4%.
  • Tests for endomysium antibodies and HLA type did not increase the PPV of samples with levels of IgA-TTG ≥10-fold the ULN.
  • Notably, 4.2% of pathologists disagreed in their analyses of duodenal morphology-a rate comparable to the error rate for serologic assays.

Effects of the addition of low-dose ketamine to propofol-fentanyl anaesthesia during diagnostic gynaecological laparoscopy

Atashkhoyi S et al. • European journal of obstetrics, gynecology, and reproductive biology (2013)

Use of low-dose ketamine with propofol-fentanyl anaesthesia in patients undergoing DGL was associated with less pain during propofol injection, lower incidence of haemodynamic changes, lower total dose of propofol and improved postoperative analgesia.

  • Patients in the study group had a significantly lower incidence of pain than patients in the placebo group during propofol injection (13% vs 87%, respectively; p<0.0001).
  • After induction of anaesthesia, 16 (53%) patients in the placebo group and three (10%) patients in the study group had a decreased heart rate (p<0.001).
  • The decrease in mean arterial pressure was greater in the placebo group compared with the study group (37% vs 7%, respectively; p<0.001).

Unmet needs of multiple sclerosis patients in the community

Lonergan R et al. • Multiple sclerosis and related disorders (2015)

Physiotherapy offers significant benefits, but is the most frequently reported unmet need. These findings highlight the need for increased fund allocation, especially for development of community supports and multidisciplinary/ social services.Identifying unmet needs may help inform health service planning, and emphasises particular need for improved resources in a high-risk group of MS patients.

  • We identified 632 patients with multiple sclerosis: 23% SCD (urban), 30.8% WEX, and 46.2% DGL.MS subtype was relapsing remitting (RR) in 51.1%, secondary progressive (SP) in 39.7%, and primary progressive (PP) in 9.2%.
  • EDSS was </=6 in 86% and >6.5 in 14%.
  • Further research warranted to confirm findings

Comparison of Quality of Carbohydrate Metrics Related to Fasting Insulin, Glycosylated Hemoglobin and HOMA-IR in Brazilian Adolescents

da Rocha CMM et al. • Nutrients (2022)

Conclusions: Among NW adolescents, GL is a better measure of carbohydrate quality, but for those with overweight/obesity, carbohydrate consumption is more associated with GC, probably because they eat/report small amounts of carbohydrates.

  • Results: Of 35,737 students, 74% were from public schools, 60% girls, 17% overweight, and 8% obese.
  • Glycosylated hemoglobin was not associated with these metrics, except for AvGL.
  • Insulin and HOMA-IR were associated with all metrics in NW adolescents, with greater coefficients associated with AvGL.

Endocannabinoid-Based Therapies

Piomelli D et al. • Annual review of pharmacology and toxicology (2022)

We briefly discuss selected agents that simultaneous-ly interfere with components of the endocannabinoid system and with other functionally related signaling pathways.

  • This signaling complex has three components: the endogenous ligands, anandamide and 2-arachidonoyl-sn-glycerol (2-AG); a set of enzymes and transporters that generate, eliminate, or modify such ligands; and selective cell surface receptors that mediate their biological actions.
  • We provide an overview of endocannabinoid formation, deactivation, and biotransformation and outline the properties and therapeutic potential of pharmacological agents that interfere with those processes.
  • We describe small-molecule inhibitors that target endocannabinoid-producing enzymes, carrier proteins that transport the endocannabinoids into cells, and intracellular endocannabinoid-metabolizing enzymes.

Study of interactions between oppositely charged dendrigraft poly-L-lysine and human serum albumin by continuous frontal analysis capillary electrophoresis and fluorescence spectroscopy

Sisavath N et al. • Journal of chromatography. A (2013)

All rights reserved.

  • For a 1 g/L concentration in G3 and assuming a plasmatic HSA concentration of 40g/L, these binding constants lead to only 5% free DGL in the medium.
  • These results are in good agreement with the presence of two negatively charged domains in the HSA.
  • Good fitting of the fluorescence spectroscopy data was obtained using the equilibrium constants derived from FACCE.

High-dose diosgenin reduces bone loss in ovariectomized rats via attenuation of the RANKL/OPG ratio

Zhang Z et al. • International journal of molecular sciences (2014)

This suggests that the anti-osteoporotic effect of DG might be associated with modulating the RANKL/OPG ratio and DG had potential to be developed as alternative therapeutic agents of osteoporosis induced by postmenopause.

  • Levels of protein and mRNA expression of osteoprotegerin (OPG) and receptor activator of nuclear factor kappa-B ligand (RANKL) in tibia were evaluated by immunohistochemistry and in situ hybridization.
  • Our results show that DG at a high dose (DG-H) had a significant anti-osteoporotic effect compared to OVX control.
  • DG-H treatment down-regulated expression of RANKL and up-regulated expression of OPG significantly in tibia from OVX rats compared to control, and thus lowered the RANKL/OPG ratio.
